We
currently provide the following services:
Description of Services
Home care aims to make it possible for people to
remain at home rather than use residential, long-term, or
institutional-based nursing care. Home Care providers render services in
the client’s own home. These services may include some combination of
professional health care services and life assistance services for
Medicaid such as:

Activities of daily living (ADLs): bathing, dressing, mobility, feeding, toileting, and incontinence.

Meal Preparation – Assisting clients with meals, helping them setup, prepare and feed if needed also cleanup of meal preparation.

Light Housework- Assisting client with daily housework such as mopping, dusting, taking out garbage, bed making, laundry etc.

Shopping / Errands – Shopping for the client, getting supplies, groceries, picking up prescriptions etc.

Medication Reminder

Laundry and Linens - Washing and folding the client’s laundry and linens and putting them in the proper place.

These services are only available for Private Pay:

New Mom’s Care - We assist new mothers post-partum with activities of daily living, personal care and also help with the baby.

Respite Care - Respite Care is a service we provide to help families or those in charge of taking care of someone by giving them a break; this service can be companion care or personal care.

Companion Care – This is non medical, we are basically a companion to the Client. We will help the client with things like getting a glass of water, playing card games, listening to music, etc. We will not administer care in any way then as a companion.

Outpatient Care
With Private Pay the following amenities are available upon request:

Buddy Call - The ACHC Buddy call is designed for the client to receive a call during business hours and after hours to check on the client’s needs and concerns
With all Private Pay
Services a Registered Nurse is on call – A nurse is available anytime
during business hours and after hours to answers important questions
that the caregiver or the client may have.
Private Pay services are not limited to these
listed above, if you have additional services you would like us to
provide please feel free to ask us about them and we can determine if it
can be added to your plan of care.

The
Qualification Criteria for Veterans or Widowed Spouses is:
The Veteran must have served 90 consecutive days
active duty with at least 1 day during war time. The Veteran must also
hold an “Honorable Discharge” status. If the Veteran had a service
related medical discharge with less than 90 days active duty and one day
during war time, he or she may qualify.
A Widowed spouse must have been married to the
Veteran at the time of the Veteran’s death and never remarried. The
Widowed spouse must have been married to the Veteran for at least 1 year
or have had children by the veteran.

Private
Insurance Companies all have their own predetermined, approved services
they will allow us to provide. Depending on which Insurance Company you
have, there may be more services we can provide or a limit to the
services we can provide. We would need to verify that information with
the insurance company prior to making a plan of care specific for your
needs.
